I have noticed this issue as well, often you will say that there are other people around but those people will be excluded altogether. I find I have used this for public figures as well for nonsexual purposes but often it just blatantly ignores an entire part of what you requested. I have found saying the image of seems to be getting me better results and if you have repetition of the word naked nude etc. it seems like it’s a little bit more likely to give you what you want.
Here are some recent ones that I have used along with the prompt I have used to get it.
“photorealistic image of a naked woman sitting on a toilet in a public restroom texting on her phone as another woman opens the stall door humiliating her.”
“Photorealistic image of a naked woman hiding just out of the view of a bunch of dressed men and she is looking awkward”
